چکیده
orthopaedic clinic with low back pain and left leg pain for last three months. The low back pain was of gradual onset and was progressive. The pain was aggravated on walking and standing. On clinical examination the patient was fit and well. She had bilateral free straight leg raise. Femoral nerve stretch test on the left leg was positive. She was intact neurologically and the range of movements in the spine were terminally restricted. Both hips and sacral iliac joints were clinically normal. A clinical diagnosis of lateral canal stenosis was made. Magnetic resonance imaging (MRI) of the lumbosacral spine revealed a well defined posterolateral extradural cystic mass (see fig 1). It had a low intensity wall signal and a high intensity content signal. Associated degenerative and spondylolisthetic spine was noted. The cyst was in continuity with the degenerative facet and gadolinium enhancement of the cyst showed a lumbar facet synovial cyst.

Outcome of percutaneous rupture of lumbar synovial cysts: a case series of 101 patients.
BACKGROUND CONTEXT Lumbar facet joint synovial cysts are benign degenerative abnormalities of the lumbar spine. Previous reports have supported operative and nonoperative management. Facet joint steroid injection with cyst rupture is occasionally performed, but there has been no systematic evaluation of this treatment option. PURPOSE To profile the role of facet joint steroid injections with ...
متن کاملSpontaneous resolution of symptomatic lumbar synovial cyst
Lumbar synovial cyst arises from the facet joint and can lead to back pain, radiculopathy, neurogenic claudication or even cauda equina syndrome. Although most surgeons would consider surgery to be the treatment of choice, the natural history of the disease process remains unknown and uncertainty still exists regarding optimal management of this controversial entity. We illustrate a case of lar...
متن کاملHaemorrhagic lumbar synovial cyst. A cause of acute radiculopathy.
A total of 254 cases of synovial cysts of the spine have been reported in the English literature, but only eight have been associated with haemorrhage. We describe a 55-year-old man with acute radiculopathy resulting from haemorrhage involving a synovial cyst at a lumbar facet joint. Traumatic factors could have caused bleeding around or into the synovial cyst. Treatment by resection of the cys...
متن کاملEpiduroscopic Removal of a Lumbar Facet Joint Cyst
Facet joint synovial cysts are usually associated with osteoarthritis of the adjacent facet joint and/or spondylolisthesis. In between the conservative and operative ends of the treatment spectrum lie minimally invasive techniques such as cyst rupture using epiduroscopy. In this report, we describe an 82-year-old male patient presenting with low back pain radiating to his lower left extremity a...
متن کاملRadiologic recognition of symptomatic spinal synovial cysts.
Synovial cysts of diarthrodial joints have long been recognized [1]. Clinically, they have most often been found in association with one of the arthritides [2,3]. Though synovial cysts can involve the facet joints of the spine, this fact was little appreciated until very recently [4, 5]. Radiographically, these lesions were extremely difficult to diagnose and were rarely recognized preoperative...
متن کاملSpontaneous resolution of an intraspinal synovial cyst.
We report a case of lumbar facet joint synovial cyst at L5-S1 with clinically significant radicular pain diagnosed by means of MR imaging. This cyst spontaneously resolved, as determined on follow-up MR images obtained 18 months later. The patient's clinical symptoms substantially improved after conservative medical treatment.
